R55 Upstream O2 Sensor

Upstream O2 Sensor
UPDATE: I took out my O2 sensor to inspect it and it was charred to a crisp. My best guess is that the excess fuel fouled the sensor. I replaced it, and the car runs like new.
I'm looking for some insight on my 2010 Mini Clubman, I've had it for almost 12 years and 120,000 miles. My upstream O2 sensor has registered two faults, P2195 and P2196 (stuck lean and stuck rich, respectively). This has been going on intermittently for a little while, and a few days ago, one of my ignition coils failed. I replaced the spark plugs and coils, and noticed a small oil leak in my valve cover gasket, so I replaced that as well. I am still getting both the rich and lean codes (I figured it might have been too rich because of excess unburnt fuel from the misfiring cylinder). The current upstream sensor (Bosch Exact Fit Oxygen Sensor 17217) only has about 5,000 miles on it, and I don't really see how both faults can be present with the same sensor. Any advice?
