Symptoms of a Clutch going

Anyone know what to look for when the clutch is starting to go? 2010 MCS with 86K currently.

You're in 5th gear, floor it and the revs go up, but speed doesn't.

Thanks! We are getting a groaning sound at odd times when shifting and accelerating.
