Belt Damper Bushing coming out

I had the OEM damper bushings replaced with powerflex ones a few months ago, the lower one had disappeared completely so I just decided to swap them out. Strangely though, the powerflex one is beginning to come out
Any ideas as to what may cause this? The belt damper was replaced a few days before the bushing disappeared(at a dealership), it's not more than a few months old (3-4 months at most) so i'm pretty sure it's not that. For some reason I feel as though the bolt head on the bottom of the piston is smaller than the top, causing it to slip out..dunno, just an idea..

That's an ATI crank pulley by the way. The bottom piston ring broke off and just scratched it, still works fine. That's why the tensioner had to be replaced. I also have a video of the car running, i'll upload it if someone feels it'll help figure things out.

Just went outside to check it out, the bolt still seems to be in there, I stuck my hand in to check and everything. The bushing seems to be going around the bolt. I tried to get a better picture of the front of the bushing, but it's a tight fit.

Contact the manufacturer and send them the pics you have posted above. They should be able to address the problem as either a defective part, a defective installation, both or something else.

It needs a washer on the bolt. for some reason there is enough pressure to force the bolt head through the bushing.

That easy.

Again, totally plausible. I'm going to try and head over to the dealership a little later. Since they swapped it out a few months ago maybe they can put a washer on there, or at least pop the bushing in properly.

Called Way (Who I got the bushings from) E-mailed him the picture, he forwarded it to Powerflex. I might have just gotten one of those "bad batch" of bushings, so I just need to wait for them to come in, and replace it, and it'll all be good as new! :D

If you need to replace them, then yeah they definitely are, they have a lifetime warranty, so i'm getting a new set for free. If you don't need to replace you're bushings then you're fine. Check your control arm bushings and such, powerflex has replacements for those as well, and a few others. Go to waymotorworks.com and you'll find them

If the bolt is still there, it sounds like whoever installed this forgot to install the washer that should have come with the bushing.
