clutch pedal fell off

Last night when I was driving back to my house I heard something metal hit the floor after shifting gears. I stopped, looked down, and found the metal cover on the clutch pedal had fallen off. What the heck happened?

Has anyone else had this problem? Gah of all the things to fall apart why this?

This happens...

I have an 02 with 56k miles. I have rubber covers (didn't get metal till 03 I think) and it fell off. Just put it back on. This happened on my Mustang as well. It will keep hapening until the pedal cover is replaced, or it's attached with something better
